Definitions:

EVIR (Employee Violence Incident Report) is violence directed at employee. The SSIR (Safe Schools Incident Report) are used for (mis)behaviours of student. EVIR/SSIR do often overlap.

The H&S Concern Forms are used for ongoing risks/hazards that should be brought to the attention of your supervisor.

The Form 140 is for injury or illness related to work. *Note that for OTs these are likely paper-based and are not in eBase. Seek one from your Administrator.


How to find eBase and report: 

    1. On the OCDSB website select Staff Portal.
    2. Enter your OCDSB credentials.
    3. Select Administration/Elementary/Secondary icon.
    4. Select Online Workplace Violence Reporting.
    5. Use OCDSB login credentials.
    6. Click  on left side, and then open a New Form in the upper right hand corner.
    7. In the Workflow box, select H&S Concern form or EVIR/SSIR.
    8. Select the work site in the Facility box.
    9. Fill out all mandatory fields, drop-downs will be created.
    10. Click Submit for review once all fields are filled out.

Your supervisor will receive an email notification once an online form has been submitted. Your supervisor will identify actions that have been or will be taken as follow up to the incident and you will receive via email the report with this section completed. The last step provides you with the option to “Mark as Unresolved/Bring to the attention of the JHSC for review” and to include additional details about the incident and any recommendations that you may have for resolving your concerns.

VIOLENT INCIDENTS ARE:

a.  Exercise of physical force that causes or could cause physical injury to the worker.

b.  Attempt to exercise physical force that could cause physical injury to the worker.

c.  Statement or behaviour that is reasonable for the worker to interpret as a threat to use physical force that could cause physical injury to the worker.

The Act does not differentiate on the basis of any physical, emotional, or cognitive disability, you must report all violence.

 

SAFE SCHOOLS INCIDENTS ARE:

Conduct that may lead to suspension or expulsion

1.  Uttering a threat to inflict serious bodily harm on another person.

2.  Possessing alcohol, illegal drugs or, unless the pupil is a medical cannabis user, cannabis.

3.  Being under the influence of alcohol or, unless the pupil is a medical cannabis user, cannabis.

4.  Swearing at a teacher or at another person in a position of authority.

5.  Committing an act of vandalism that causes extensive damage to school property at the pupil’s school or to property located on the premises of the pupil’s school.

6.  Bullying.

7.  Any other activity that is an activity for which a principal may suspend a pupil under a policy of the board.

OR

Conduct that shall lead to suspension or expulsion

1.  Possessing a weapon, including possessing a firearm.

2.  Using a weapon to cause or to threaten bodily harm to another person.

3.  Committing physical assault on another person that causes bodily harm requiring treatment by a medical practitioner.

4.  Committing sexual assault.

5.  Trafficking in weapons or in illegal drugs.

6.  Committing robbery.

7.  Giving alcohol or cannabis to a minor.

7.1  Bullying, if,

i.  the pupil has previously been suspended for engaging in bullying, and

ii.  the pupil’s continuing presence in the school creates an unacceptable risk to the safety of another person.

7.2 Any activity listed in the ‘may suspend‘ list above that is motivated by bias, prejudice or hate based on race, national or ethnic origin, language, colour, religion, sex, age, mental or physical disability, sexual orientation, gender identity, gender expression, or any other similar factor.

8.  Any other activity that, under a policy of a board, is an activity for which a principal must suspend a pupil

These shall be reported to the principal as soon as reasonably possible. The employee must consider the safety of others and the urgency of the situation in reporting the incident, but, in any case, must report it to the principal no later than the end of the school day. Refer to Education Acts.300.2.
